Stay Healthy Controlled Substance (Prescription) Refills/Renewals Policy
A controlled substance is a chemical that is that is regulated by the government for its manufacture, possession, and use. These substances are tightly controlled by the government because they may be abused or cause addiction.

You must agree to a urine drug screening if requested. Substance use of any kind may result in termination of medication management services by this office.

Regular office visit follow-up for periodic reassessment is expected as part of the treatment plan.

Renewal process: Requires a visit with the prescribing provider to renew the prescription.

Renewal of controlled substances can only occur during normal business hours.

Use of only one pharmacy for renewal of controlled substances. Please call ahead for renewal requests.

Controlled substances will not be mailed to pharmacies.

Controlled substance prescriptions will only be submitted electronically.

Sharing of medication or persion of medication for other than indicated purposes can result in felony prosecution.

Patients must inform the office if they use any other controlled substances or pain medications prescribed by another provider.

There can be no use of any other controlled substance or pain medications prescribed by another provider without informing this office.

Only the person prescribed the medication can pick it up after verifying their identity.

No emergency prescriptions will be provided for lost or stolen medications. We advise that you store your medication in a locked cabinet or drawer, and away from children or pets.

If my medications are stolen, the provider may refill the prescription, one time only if a copy of the police report of the theft is submitted to the office.
All patients of Stay Healthy must understand and agree to the Controlled Substance (Prescription) Policy.
The patient is responsible for making sure that they do not run out of medications on weekends and holidays, because abrupt discontinuation of certain medications can cause severe withdrawal syndrome.
The patient agrees to refrain from all mind/mood altering/illicit/addicting drugs including alcohol unless authorized by my provider.
